Annual Sworn Statement Online Forms

There are two forms below: 1)  Annual Sworn Statement - Non-Corporate Medallions and 2) Annual Sworn Statement - Corporate Medallions. 

All individual Medallion Holders must complete the "Non-Corporate" form. Only Corporate Medallion Holders must complete the "Corporate" form. Please scroll to the bottom for the "Corporate" form.

Annual Sworn Statement - Non-Corporate Medallions

Note: All fees are proposed to be waived in the SFMTA budget which is pending final approval.


You will need the following information to fill out this form:

  • A Driver's License or ID Number
  • An electronic copy of your Driver's License or ID to be uploaded

Annual Sworn Statement - Corporate Medallions

Note: All fees are proposed to be waived in the SFMTA budget which is pending final approval.


You will need the following information to fill out this form:


  • CA Secretary of State (SOS) File Number (Form SI-200C)
  • An electronic copy of your Driver's License or ID to be uploaded
  • A stamped copy of Form SI-200C from the California Secretary of State for the previous calendar year 
  • Stockholders Information for Corporation
