TO:                  San Francisco Taxi Drivers, Color Scheme Managers and Medallion Holders

Pursuant to San Francisco Transportation Code, Division II, §1105(a)(5), “[u]nless earlier revoked or suspended, all permits shall expire one year following their issuance or renewal, or on another date as specified by the SFMTA.”  All San Francisco Taxi Medallions are set to expire on Tuesday, June 30, 2026.  To ensure that your Medallion is renewed in a timely fashion, and to prevent interruption in service, please ensure that all requirements for renewal are complete: 

 

Medallion Annual Sworn Statement Online Forms 

 

Transferable (Purchased) Medallions: 

(1)   Sign a statement under penalty of perjury affirming eligibility for the Transferable Medallion on a form provided by SFMTA 

(2) Submit a copy of valid identification 

 

Post-K Medallions: 

(1)   Pay the applicable Renewal Fee 

(2)   Sign a statement under penalty of perjury affirming eligibility for the Post-K Medallion on a form provided by SFMTA 

(3)   Submit a copy of a valid California driver’s license 

(4)   Maintain a valid A-Card 

 

Pre-K Medallions: 

(1)   Pay the applicable Renewal Fee; and 

(2)   Sign a statement under penalty of perjury affirming eligibility for the Pre-K Medallion on a form provided by SFMTA. 

(3) Submit a copy of valid identification 

 

Pre-K Corporate Medallions: 

(1)   Pay the applicable Renewal Fee; 

(2)   Sign a statement under penalty of perjury affirming eligibility for the Pre-K Corporate Medallion on a form provided by SFMTA; 

(3)   Copy of current stock register;  

(4)   Copy of current filed copy of Statement of Domestic Stock Corporation issued by the California Secretary of State. 

(5) Submit a copy of valid identification
