Complete the Pavement Restoration-Restriping Request Form
On SFMTA Striping Drawing (or map of the work area), Identify the areas requiring restriping.
If available, photos of the work area may also be sent with these documents
Email all to trafficpermits@sfmta.com with the subject field "Pavement Restoration"
