Regional Transit Connection (RTC) Card Request

NEW! If you are an existing RTC customer with a permanent disability (designated by a red "P" on the card) you can submit your renewal request using the online form below. No fee or in-person visit required. For all other renewals call 415.646.2224.  

New medical applications are accepted at the SFMTA Customer Service Center (11 South Van Ness Avenue/Monday - Friday from 8:00 A.M. - 5:00 P.M). Please bring a completed application, a $3 processing fee, and a valid government-issued ID.


Please fill out the form below to request a renewal or replacement of your RTC Discount ID Card. For more information about the RTC program, please visit the Metropolitan Transportation Commission's RTC program page.

All fees for renewal and replacement card applications received online will be waived for San Francisco residents only.
