Submittal of Purchased Medallion Beneficiary Form

Effective October 1, 2026, Purchased Medallion Beneficiary Forms will only be accepted through one of the following two methods:

1. In Person: Form completed on-site by the medallion holder at the Taxi Services Window. Notarization not required for in-person submissions.  
2. By Mail: Form completed outside the Taxi Services office must be notarized. The original notarized form must be mailed to:
