Important

To request a Hearing (Level 2 Protest) for a Speeding Ticket using the Printed Paper Form, the following are required:

  1. Receive results in the mail from a completed Admin Review (Level 1 Protest)
  2. Within 21 days of receiving the results:
    1. Submit the Hearing (Level 2 Protest) Request Form using a method listed below.
    2. Include Payment of the fine shown on the ticket(s), or if unable to pay, submit the low-income payment waiver form for approval.

Reminder: You must complete a Admin Review (Level 1 Protest) and receive results in the mail before starting the hearing process. There is no way to skip the Admin Review.

How to Submit the Hearing Request Form

You can submit the Hearing Request Form by Mail or by hand delivering it to the SFMTA Customer Service Center.

Important

Hearing Requests using the printed Hearing Request Form are not accepted by any other methods.

Hearing Request Form By Mail

  1. Download and complete the Hearing Request Form:
    Speeding Ticket Administrative Hearing Request Form - English
     
  2. Prepare your Payment or Low-Income Waiver Form
    1. Include check or money order payable to “SFMTA – ASE Program”, 11 South Van Ness Ave, San Francisco, CA, 94103. Put the Notice # on your payment.  DO NOT MAIL CASH.
    2. If you can't afford to pay fine, submit the low-income discount form for approval to waive the payment. Fine payment is required for denied low-income discount forms
       
  3. Mail the form and payment or waiver to:

Important

If you’re not approved for the payment waiver, you must pay the ticket fine.

Hearing Requests without payment of fine or approved low-income discount form are subject to rejection.

 

Hearing Request Form By Hand Delivery

  1. Download and complete the Hearing Request Form:

  3. Drop off the form and payment or waiver Monday through Friday, 8:30 AM to 5:00 PM
    Note: The Customer Service Center is closed on federal and state holidays

If you can't afford to pay the ticket fine, get the "Low-Income Discount Form" to see if you qualify for a payment waiver and reduced ticket fine.
